I don't have a Doctor's order for the services that I need. I am not even sure if I qualify for services. How can Cole Home Healthcare help me?

We provide free, in home assessments to determine the best services to provide to individual patients, and partner with Physicians to provide the most efficient care possible.

What is the best way to refer patients to your Agency?

We support many options for referrals. These include direct fax to our Agency at 281.376.4357 (Help) as well as calling us directly at 281.379.7052 and asking for the Intake Department. How do your services differ from other Agencies?

Without giving out trade secrets specific to our Organization, we can honestly say that we offer many programs that most Agencies either can't, or won't offer. Please call or email us for more specifics that are applied directly to the best possible outcomes for our patients.

We see patients across Greater Houston and the surrounding counties. At Cole Home Healthcare, we believe that each patient deserves respect, dignity, and individualized care. That is why we handle each patient as an individual case and respond to them appropriately.

Who qualifies for Home Health Services?

According to Medicare guidelines, individuals qualify for home health if:

1. Your doctor decides you need medical care in your home, and makes a plan for your care at home, and 
 
2. You need at least one of the following: intermittent (and not full time) skilled nursing care, or physical therapy or speech language pathology services, or a continued need for occupational therapy, and 
 
3. You are homebound. This means you are normally unable to leave home and that leaving home is a major effort. When you leave home, it must be infrequent, for a short time. You may attend religious services. You may leave the house to get medical treatment, including therapeutic or psychosocial care. You can also get care in an adult day-care program that is licensed or certified by a state or accredited to furnish adult day care services in a state, and 
 
4. The home health agency caring for you must be approved by the Medicare program.

What exactly does Home Health Services Involve?

Any Skilled Nursing, Physical Therapy, Occupational Therapy, Speech Therapy, Medical Social Worker or Home Health Aide services that they patient may need as ordered by their Physician.

What does Medicare cover in regards to Home Health Benefits?

Medicare covers Home Health Services at 100% providing that that patient has been deemed Homebound, and their Physician orders such services.

Who should consider Home Health Services?

You may consider Home Health Services if you need assistance caring for yourself due to an illness, disability, surgery, recent hospital stay or you are simply less able due to age.

Does Medicare cover the purchase of Home Medical Equipment?

Medicare may pay for your needed medical equipment.  Cole Home Healthcare will coordinate this and ensure that you are provided with the full benefits of which Medicare allows.

What if I need assistance after business hours or over the weekend? 

We have a nurse on call 24 hours a day, 7 days a week.  Should you need help at anytime, you may call 281.379.7052 as speak with a nurse.
