There are 2 quick ways to inquire about services at Cole Home Healthcare. You may do so one of the following ways:

1. Call us at 281.379.7052 to speak with our intake coordinator.
2. Simply fax the patient demographics and physicians order to 281.376.4357, Attn: Intake.
3. Download and complete the Intake form below. Please fax to 281.376.4357, Attn: Intake.

Within 2 hours of receiving and confirming the referral, a staff member from our office will contact you and will have taken the appropriate measures to do the following:

• Spoken with the patient/family to ensure that they want to receive home health services.
• Verified Medicare and/or Insurance.
• Spoken with the treating therapist/ nurse to schedule a time to do the evaluations and treatments.
• Confirmed with the referral source that we will be treating the patient.

If for any reason, we are unable to service the patient, we will assist you in finding someone who will!
